Brooke Mueller's whereabouts -- and the state of her health -- have finally been confirmed. "Despite reports to the contrary, Brooke is currently at UCLA Medical Center where she has been since being admitted May 3," a rep for Mueller, 35, tells Us Weekly. On Friday, news broke that Bob and Max, Mueller's 4-year-old sons with ex Charlie Sheen, had been removed by the Los Angeles County Department of Children and Family services after deeming the residence an "unsafe environment."

Rumors circulated that Mueller -- who has struggled with substance abuse before -- had been admitted to a psychiatric facility. Clarifies her rep: "Brooke will be [at UCLA Medical Center] for several more days, and will likely transition directly into a residential treatment facility to get help for prescription drug abuse."

In a talked-about arrangement, Bob and Max are currently staying with their father's second ex-wife, Denise Richards. "Brooke misses her children, and wants to get healthy so she can resume her life and get back to being a mom," the rep continues.

Mueller's most recent rehab stay -- her 19th -- was back in December of last year, where she was treated for an addiction to Adderall.

No stranger to addiction struggles himself, Sheen, 47, said (via his rep) that he "supports the actions of the Department of Children and Family Services," and that he would "fully cooperate and fully participate" in any future proceedings. "He knows Max and Bob are safe in a stable, loving environment with Denise and the boys' sisters."

"They've been under her care many times before," an insider told Us. "This isn't unusual. Truthfully, Denise and [her daughters] are the only consistency those poor kids have had. [Bob and Max] are her family, and Denise would do anything for them."
