On May 16, 2012 in NYC, Michele and Monteith packed on the PDA during the New York Rangers vs. New Jersey Devils playoff game at Madison Square Garden. "I saw some photos and I was like 'Damn! We look good,' " Michele told E! News of how their Glee sex scene turned out. "As a fan of the show, it's something I wanted to see for awhile and it's very interesting how everything plays out."

During a March 19 outing, Michele wore a gold necklace baring Monteith's name. Just a few days later on April 1, Monteith's rep confirmed to Us that the actor "voluntarily admitted himself into a treatment facility for substance addiction." In her own statement, Michele stood by her man, saying in her own statement: "I'm grateful and proud he made this decision."

After Monteith's month-long rehab stint, the couple kicked back in Puerto Vallarta, Mexico, on May 7, enjoying some much-needed R&R together. "Sailing off on another adventure… Putting down the electronics for the next few days.. Gonna rest up mind body and soul! See u soon!” Michele tweeted of the vacay. Monteith seemed just as happy, tweeting a thank you to fans post-rehab. "Sending out big love to everyone," he wrote. "Thank you for the continued support! It means the world to me!"

On May 11, the pair celebrated his 31st birthday at a private club in West Hollywood. "A very happy birthday to the best guy I know Mr. Cory Monteith," Michele tweeted. "Thanks for making me feel like the luckiest girl in the world every day." For his part, he wrote: "Thanks for all the birthday wishes! It was a great day. Officially old now."

On June 18, the Glee stars were seen walking around in the SoHo neighborhood in N.Y.C. An insider told Us at the time that the pair were "going strong" following Monteith's rehab stay. "Everything is good," the source added.

Tragedy Strikes

On Saturday, July 13, police announced during a press conference that Monteith was found dead in a hotel room at the Fairmont Pacific Rim Hotel in Vancouver, Canada. "We are so saddened to confirm that the reports on the death of Cory Monteith are accurate," the actor's rep said in a statement to Us. "We are in shock and mourning this tragic loss." Michele didn't have a comment, but a rep for the actress told Us: "We ask that everyone kindly respect Lea's privacy during this devastating time."
