Law enforcement responded to Hayden Panettiere‘s West Hollywood apartment for a welfare check one week before the actress’ death.

Us has exclusively obtained a call report from the Los Angeles County Sheriff’s Department revealing that someone requested a welfare check on a woman at the residence who was not answering phone calls.

The heavily redacted report did not identify Panettiere, but a source with knowledge of the situation confirms the welfare check was for the Nashville star. “She had become unresponsive to many people in her circle and there was concern,” the insider tells Us.

Per the log, the caller was “concerned” that the unidentified white female was “not answering the phone.”

The 911 call was made at 10 p.m. on August 9, just one week before Panettiere was found dead in a South Carolina Airbnb. She was 36.

When officers responded, they were informed that “[redacted] was sick all day and did not feel good.” The source says Panettiere’s on-again, off-again boyfriend, Brian Hickerson, was allegedly the one who answered the door.

The source further claims that “after being hospitalized a week before her passing,” the actress “informed a friend who came to see her that Brian had taken her phone.”

According to the record, law enforcement reported no other follow-ups.

The call for service report also showed that sheriff’s deputies responded to Panettiere’s L.A. home two additional times in 2025.

On August 4, 2025, at 11:47 a.m., officers were alerted that a white male wearing a green polo, sweats and no shoes was on the third floor of the complex trying to enter a unit.

By the time police arrived at 12:15 p.m., the male had “left the location.” The report noted it was an “ongoing issue.” Authorities appeared to advise that the caller “change [the] locks” and notify them if the unidentified male returned.

Just hours later, at 3:50 p.m., the same individual was reported going in and out of the apartment complex, according to the report. At 4:15 p.m., arriving officers noted that the disturbing party “has moved out temporarily while they remodel” the apartment. They also advised the unidentified caller about “letter of agency” procedures, which is a legal document signed by a property owner or manager giving law enforcement pre-authorized permission to enter private property to enforce trespassing laws and arrest unauthorized individuals.

Officers were called to the apartment again at 12:22 p.m. on September 8, 2025, regarding an “ongoing issue” with an unidentified white male. The unknown caller told authorities that the man, who was wearing a green shirt and tan or gray pants, was in the building’s garage and “refused to leave.”

Less than two hours later, sheriff’s deputies made contact with a white male and white female and reported that the disturbing party “grabbed some belongings then left.” Law enforcement also advised “alternate options for resolution.”

Us reached out to Panettiere’s rep and Hickerson’s lawyer for comment.

Following news of the Heroes star’s death, TMZ reported that Panettiere’s neighbors had previously been instructed to call 911 if Hickerson, 37, was spotted on the property. (Hickerson was not identified in the redacted report and it’s unclear whether he was involved.)

According to a neighbor, the homeowner’s association posted flyers around the complex at the end of August 2025, urging residents not to let Hickerson into the building and to inform officers if they saw him around.

The neighbor told the outlet that the HOA removed the flyers and warnings after about a week, once Hickerson was no longer considered a threat.

As Us previously reported, Hickerson was arrested twice in August 2025 — the first time at Barney’s Beanery on August 2 and again on August 6.

The L.A. County Sheriff’s Department confirmed to Us that charges for both arrests were “under the influence of a controlled substance,” but prosecutors declined to prosecute Hickerson due to lack of evidence. Hickerson entered a diversion program, and after he completed 10 Narcotics Anonymous sessions, the judge dismissed the case. (People was first to report the news.)

Panettiere was found unresponsive on August 16 in an Airbnb that she was sharing with Hickerson in South Carolina. Hickerson and his brother, Zach, performed life-saving measures until help arrived, but paramedics were unable to revive the actress.

The Greenville County Coroner recently revealed Paniettere’s death was caused by “toxic effects of fentanyl, 4-ANPP, Alprazolam, Methocarbamol, and Quetiapine,” ruling her death as an accident. An investigation into Paniettere’s death and the source of the drugs remains open and active.
