The TLC series’ preview clip shows Matt, 54, meeting with Dr. Kent, following his health scare in the previous episode. Viewers watched last week as Matt underwent a procedure on his neck, only to learn that he would likely need a more serious surgery down the road.

“Since my neck’s getting worse, I have an appointment with the doctor to get a second opinion,” Matt tells the camera in the clip. “It’s causing me to slow down and have to cut my activities down a lot.”

During the hospital visit, the specialist tells Matt in no uncertain terms that he could face permanent paralysis if he continues to avoid surgery.

“Dr. Kent put it quite bluntly and reinforced what the other doctors have said, which is that if the spinal cord gets damaged or severed, it’s going to make some radical changes in my life in a hurry,” he continues. “This is serious business. I knew that I was going to need surgery soon. Turns out, I can’t wait — it’s like walking around like a time bomb. I mean, even the slightest mistake can leave me paralyzed, so it’s pretty scary.”

The medical issue comes as the farmer and estranged wife Amy continue to deal with the drama from their painful split. (The pair finalized their divorce in May, which has not yet happened on the show.)

Little People, Big World airs on TLC Tuesdays at 9 p.m. ET.
