Brooke Mueller has again fallen on hard times.
Charlie Sheen's ex-wife, 34, has checked into rehab for substance abuse for the second time in six months, Us Weekly has confirmed.
Voluntarily checking into an unnamed facility last month, Mueller's relapse follows a December drug arrest in Aspen, Colo. Following her arrest, the mom of 3-year-old twins Bob and Max completed a 90-day rehab stint, and was released less than two months ago.
In a statement, Mueller's rep, Steve Honig, said Sheen's troubled ex is committed to getting clean. "As part of her ongoing treatment and as planned, she voluntarily checked herself into a rehab facility several weeks ago," Honig tells Us Weekly. "Brooke is doing well and is grateful she has a strong support system on which she can rely."
Though TMZ reports Sheen attempted to get his ex examined by an addiction specialist, Celebuzz says the Anger Management actor played a different role in her latest downward spiral.
According to a Celebuzz source, Mueller "met up with Charlie again, partied for four days straight at his Mulholland Estate home and ultimately fell off the wagon."
Finalizing her divorce from Sheen in 2010, Mueller has retained custody of their sons since June 2011.
