An Israeli model known for her high-profile relationship with actor Leonardo DiCaprio, Rafaeli was born in 1985 in Hod HaSharon, Israel. Embarking on her modeling career as a baby, the brown-hair, blue-eyed beauty has appeared on the covers of Sports Illustrated, Maxim, and international versions of Elle and GQ.
Dating DiCaprio since 2006, the couple split for six months in 2009 before rekindling things the following year. They split again in spring 2011.
